Service provider Connect Health has commissioned Healthwatch South Tyneside to carry out the research to find out how users rate access, communication, involvement in their own care plans and their appointments.

The team includes specialist clinicians covering all musculoskeletal areas, who provide a comprehensive assessment and a range of treatments and management plans. They treat MSK conditions involving bones, joints and soft tissues.

Healthwatch South Tyneside staff and volunteers are attending two of the South Tyneside Community Musculoskeletal Service clinics this month at Cleadon Park Primary Care Centre and Flagg Court Primary Care, where patients are being invited to complete questionnaires.

We also want to hear from other users who have had appointments at Cleadon Park Primary Care Centre or Flagg Court Primary Care Centre.

The survey can also be completed online here until January 23. If you require support completing this survey or have any questions, please get in touch by emailing info@healthwatchsouthtyneside.co.uk or calling us on (0191) 489 7952.

Connect Health has been a leading provider of NHS community services since 1993, including musculoskeletal conditions, chronic pain, orthopaedics and rheumatology.
