NHS England has developed a survey for individuals with Coronavirus (Covid-19) to share their experience of the virus.
The survey will help the NHS understand more about how and where Covid-19 has affected people and is affecting people currently.
By answering some quick questions, you can help the NHS to help people with the virus.
The answers you give will not be shared beyond the NHS and trusted organisations working directly with the NHS in response to Covid-19.
Please share the survey with family and friends to help spread the word, to ensure that as many people as possible respond. The more people the NHS hear from, the bigger evidence base they will be able to gather to help their fight against Covid-19.
